Methods : Searches related to vibration therapy research were conducted in PUBMED using “Vibration”, “Whole Body Vibration”, “Localized Vibration”, and “Focal Vibration”. The Conditions were limited to review and systematic review. Results : Roberto Casale's paper was selected as an inquiry task and reviewed critically and narratively by referring to other papers. The stimulation process of local vibration (LV) was broadly classified into receptor transmission (pain reception phase), ascending sensory pathway to the spinal cord (segmental phase), and action of the cortex and subcortical structures (systemic control phase) according to the pain pathway. In addition, the role of C-tactile mechanoreceptors, changes in neurotransmitters and neurohormones, LV stimulation below perception threshold (lower threshold), pain control and kinesiologic illusions were specially addressed. In addition, the expression and function of Piezo Channels were added to supplement the human pain and tactile sensing mechanism. Conclusions : LV exerts pain control mechanisms through different interactions that can interfere with pain transmission and pain perception. Since LV provides sufficient neurophysiological reasons for clinical application, it is necessary to expand the use of Whidam’s vibrator Sugi therapy to a wider range of clinical applications. * Keywords : Whidam’s Sugi therapy, Whidam’s vibrator Sugi therapy, Medical Gigong, Vibration, Neurophysiology [목차] Ⅰ. 緖論 Ⅱ. 硏究對象 및 方法 Ⅲ. 結果 Ⅳ. 考察 Ⅴ. 結論 Ⅵ. 參考文獻 [서론] 휘담식 수기요법은 의료기공 수련법에서 유래한 인체관을 바탕으로 기 흐름의 편차현상인 적취(積聚) 해소를 목적으로 시술자의 손과 기(氣)를 이용하여 인체의 체액조절과 전신조정의 방법으로 기를 조절하는 치료법으로 제안되었다[1]. 휘담식 수기요법은 의료기공의 인체관과 병리관을 적용해 조기치신(調氣治神)의 효과를 유도하는 치료법으로 의료기공의 임상 적용 방법론이다[3]. 휘담식 수기요법의 주요 특징은 적취를 치료 대상으로 하는 적취 이론이다[1]. 적취는 사기(邪氣)의 응결체로[3] 압력과 마찰력, 유도력으로 고정된 자리에서 움직여서 풀어내면 적이 취가 되고 사기가 정기화 되면서 치료효과가 발휘된다[1]. 적취를 풀어내기 위해서는 수련한 한의사가 내기(內氣)를 사용해 손끝에서 발출되는 외기(外氣)를 사용해야 하는데[3] 시술 시 적취를 탐색할 때 한의사의 정기(正氣)와 환자의 사기가 충돌하여 통증이 유발된다[1]. 이로 인해 통증에 민감한 환자인 경우 접촉치료에 어려움이 있을 수 있다. 통증에 민감한 환자에게도 기공원리를 구현하면서 접촉 시술할 수 있는 여건이 필요하게 되었다. 그 노력 중에 하나로 진동기 수기요법이 제안되었다[4]. 진동기 수기요법은 휘담식 수기요법의 시술 기법 중 진법(振法)의 기법을 좀 더 용이하고 효과적으로 사용하기 위해 전동식 진동기계를 활용한 것이다. 진동치료는 치료 적용 부위에 따라 전신진동(Whole body Vibration: WBV) 운동과 국소진동 또는 초점진동(Local Vibration or Focal Vibration: LV) 자극으로 나눠 볼 수 있다[5]. WBV는 근육 강화를 위한 방법으로 다양한 임상 상황에서 점점 더 많이 사용되고 있다. 다양한 장애에서 진동 치료의 장기적인 사용에 대한 대부분의 연구는 근력 증가, 균형 개선, 골량 증가라는 세 가지 치료 목적을 추구했다[6]. 그러므로 WBV는 신경변성, 근손실, 골다공증, 통증에 효과적인 운동 전략으로 여겨진다[7]. LV는 잘 견디고 효과적이며 사용하기 쉬우며, 신경 병리학의 병인학과는 무관하게, 뇌졸중, 척수 손상, 다발성 경화증, 파킨슨병 및 근긴장이상증(Dystonia)과 같은 신경학적 질병 또는 장애를 포함한 신경재활에서 기능활동 뿐만 아니라 보행 훈련에서도 운동 활동과 운동 학습을 촉진하고 경련을 줄이는 데 사용될 수 있다[8]. 진동치료는 이와 같은 이유로 국내의 경우 근력향상 목적의 WBV에 대한 연구는 활발한 반면 통증제어, 신경회복 목적의 LV에 대한 연구는 상대적으로 적다[5]. 진동치료의 기전으로 긴장성 진동 반사(tonic vibration reflex : TVR) [9] 이론이 많이 알려져 있으나 최근 인간의 촉각과 통증에 대한 연구는 엄밀한 과학이론으로 발전해 가고 있어 진동치료의 기전 탐구에도 엄밀함이 더욱 필요하다고 사료된다. 이에 저자는 휘담식 진동기 수기요법의 치료 원리와 기전을 연구하면서 LV의 기전을 검토하였고 최근 연구에 대한 정보를 얻어 공유하고자 한다. 덧말
