Methods : We used combinations of korean medicine such as herbal medicine, acupuncture, cupping therapy, and thermal therapy. At the same time, the patient was treated with Whidam’s Vibrator Pelvic Sugi Therapy. The first period of hospitalization was 14 days and the second period was 54 days. At the second hospitalization, back pain recurred and was unable to walk. At the first hospitalization, L5-S1 HIVD and L4-5 Mild HIVD were diagnosed on lumbar MRI, and there was no abnormality in blood tests. Results : By relieving muscle tension and restoring the weakened muscle elasticity with herbal treatment and Whidam’s Vibrator Pelvic Sugi Therapy, back pain decreased and the inability to walk gradually improved, making it possible to walk on its own. Conclusions : The results of standing and walking on one's own were obtained by relieving muscle tension and restoring weakened muscle elasticity in acute back pain syndrome with gait disturbance diagnosed with lumbar herniated intervertebral disc through korean medicine treatment and Whidam’s Vibrator Pelvic Sugi Therapy. Whidam’s Vibrator Pelvic Sugi Therapy is thought to help acute back pain by restoring muscle elasticity and strengthening muscle strength. In the future, it is expected that additional clinical studies will be conducted on various pain diseases with Whidam’s Vibrator Pelvic Sugi Therapy. * Keywords : Whidam’s Vibrator Pelvic Sugi Therapy, Acute Back Pain Syndrome, Lumbar Herniated Intervertebral Disc, Gait Disturbance [목차] Ⅰ. 緖論 Ⅱ. 本論 Ⅲ. 考察 Ⅳ. 結論 Ⅴ. 參考文獻 [서론] 급성 요통 증후군은 급성 요추부 염좌라고도 불리며, 추간판 탈출증이나 골절 등 명백한 해부학적 이상이 있는 상태나 급성 염좌 등 이환기간이 3개월 이전인 경우로 병리해부학적 이상상태 없이 3개월 이상 허리부위에 지속되는 통증을 지칭하는 비특이적 만성 요통 증후군과 구분된다. 심평원에서 제공하는 2021년 보건의료빅데이터개방시스템에서 다빈도빌병 통계를 조회하면 요추 및 골반의 관절 및 인대의 탈구, 염좌 및 긴장은 한방 입원환자수와 외래 환자수에서 각각 2위를 차지하는 질환이다. 단순 염좌일 경우에는 얼음찜질, 적절한 휴식, 물리치료, 약물치료 등으로 빠른 시간 안에 회복시킬 수 있으나[1], 단순 염좌가 아닌 추간판 기원성 급성 통증일 경우에는 추간판 탈출증에 준하여 급성기에 보다 적극적인 보존적 치료가 필요할 수 있다[2]. 요추 추간판 탈출증은 추간판의 퇴행성 변화나 외력에 의해서 섬유륜의 파열이 발생하여, 수핵의 일부 또는 전부가 파열된 섬유륜 사이를 뚫고 외부로 탈출하는 질환을 말한다. 탈출한 수핵이 경막이나 신경근을 자극하여 통증과 함께 신경학적 이상을 유발하는 질환을 말한다[3]. 한의학적 치료법으로는 내상, 외상, 육음을 감별하여 침, 뜸, 한약, 약침, 추나, 매선, 부항 등의 보존적 치료를 적용할 수 있으며 그 중 추나요법은 단독치료(A/High), 병행치료(A/Moderate)를 권고한다[4]. 수술적 치료와 보존적 치료 사이에는 큰 차이가 없는 것으로 보고되고 있으며[5] 침구치료, 한약치료, 한방물리치료 등의 한의학적 치료는 전체적으로 치료율이 우수한 것으로 보고되고 있다[6]. 본 연구에서는 급성 요통 증후군으로 한방병원 외래 내원하였다가 근력 약화로 보행이 어려워 입원조치한 환자에 대하여 추간판탈출증으로 진단하고 근력 개선 및 통증 완화를 목적으로 휘담식 진동기 골반수기를 시술하여 유의미한 결과를 얻었기에 이에 보고하고자 한다. 덧말
